Welcome Home Education Families!

Toledo Public Schools has gathered all of the required Home Educations forms in this central location so you can easily print off, complete the forms and return them to the Home Education Office at the TPS Educational Campus, 1609 N. Summit St.

Employees in the Home Education Office are available to provide support in accessing the forms if you need such help and are available to answer any questions you may have.

Special COVID-19 Message:
Ohio lawmakers took into consideration the impact of the COVID-19 pandemic on home-educated students and passed House Bill 197, which extends the deadline to December 1, 2020 to turn in the required academic assessments. Toledo Public Schools cannot excuse your child or children from attendance laws for traditional schools unless we receive your academic assessment on or before December 1, 2020. Any assessments not received by this date Dewill result in the student(s) being considered truant and our truancy process will be enacted, which may include court referral for non-attendance will be pursued. An assessment form is not required for kindergarteners. Please note that the Home Education Notification form is due by Friday, August 21, 2020 and the Academic Assessment is due by Tuesday, December 1, 2020. Also note that the Home Education approval is conditional, pending the final results of the assessment; the approval can change in December of 2020 IF the assessment does not indicate proper progress.
